This is the only application required for most vendors working on GORDON and/or GILLILAND. Vendors who will work in a specific area of the ship during regular working hours, when the ship is moored, and when crew members are available to supervise the work need only complete this application. Exceptions may apply. Vendors should check with the Port Engineer concerning whether the below application is required IN ADDITION to this application. We recognize that the applications contain similar data, however, both may be required. We appreciate your patience in meeting increased requirements for Port Security.

This applies only for vendors who will sail with the vessel or who will work without direct supervision. Please check with the Port Engineer to determine if this application is required. If required this is IN ADDITION to the LMSR Access Application (above) which is required for all LMSR Vendors.
